EE climbed down from dozer and he forgot his cell phone, as he stepped on push beam to climb back up, his foot slipped and his leg went between push beam and track frame and he twisted his right knee.

CO-WORKER WAS SWINGING A 20 LB SLEDGE HAMMER WHEN THE SLEDGE CAME OFF THE HANDLE AND BOUNCED OFF THE TIRE THEN FELL TO THE GROUND STRIKING INJURED ON THE LEFT FOOT, BREAKING THE BIG TOE.
