EMPL WAS OPERATING A FRONT END LOADER AT THE BAUTISTA PLANT AFTER LOADING HIS TRUCK THE DRIVER STARTED DOWN THE LADDER AND HIS FOOT SLIPPED OFF THE STEP AND THE DRIVER FELL TO THE GROUND DRIVE R COMPLAINED ABOUT PAIN IN HIS BACK AND WAS TRANSPORTED TO HOSPITAL VIA AMBULANCE
