There was two other miners on-site and neither of them saw what happened. They found ee lying down next to the water truck and the rock crusher water tank. He was bleeding from the back of his head.

Conditions of the mine site had not changed since the last MSHA inspection. Conditions were normal. The severity of the operators injury consists of a severe laceration to the tip of his left index finger. Stitches were required.
