EMPLOYEE PULLED HIMSELF UP ON THE SIDE OF THE TRUCK TO SIGN OUT THE DRIVER AND WHILE DOING SO BURNED HIS RIGHT FORARM ON THE EXHAUST STACK. NOFIRST AID WAS REQUIRED.

EMPLOYEE WAS SHOVELING HARDENED FLYASH FROM BED OF DUMP TRUCK AND PULLED MUSCLE IN UPPER BACK/SHOULDER AREA. (THIS ACTIVITY AGGRAVATED A PRE-EXISTING CONDITION) THE HARDENED ASH WAS A RESULT O F HIGHER THAN NORMAL ASH-WATER MIX IN TRUCK BED.

DRILLING TEST SOIL SAMPLE, SUB CONTRACTOR EE WAS SCREWING AN ADAPTER CAP ONTO DRILL RODS. RODS SUDDENLY DROPPED AND CAUGHT EMPLOYEE'S LEFT HAND BETWEEN ADAPTER CAP AND TOP OF AUGER. LEFT HAND WAS PINCHED AND MASHED CAUSING LACERATION OF HAND.
