Associate was exiting the primary crusher area after replacing toggle plate and seat. While crossing over an I-beam, he tripped and fell fracturing his wrist.

Muddy shoes contributed to slip resulting in laceration to shin on right leg.

Associate descended ladder on the front of the Cat water truck. Upon reaching the bottom and dismounting he placed his feet on a wash out hose. Since he was on uneven footing he rolled his ankle causing a sprain.

Assoc was greasing at secondary. Had grease line in his hand and hose was going to the bearing. Bearing would not take grease and blew crease out - cutting rubber glove and small laceration to finger.

Associate pulled a muscle in his back while operating a chain jack. His torso was twisted slightly and his back was misaligned, while he was pulling on the chain jack handle.

Employee was using long bar to dislodge a slab of material from Lokolink 1, rock rolled down conveyor, pinching right pinkie finger between bar and rock, 2 stitches required.

Associate is Quality Control lab technician, he was carrying a bucket out the lab door to empty it, stepped down and his right knee twisted and popped.

Associate descending stairs from tertiary plant, stepped onto ground twisted ankle, upon further doctor care, found chipped bone in ankle.

ASSOC. STRAINED CHEST MUSCLE WHILE LIFTING SPOOL OF CABLE INTO MANLIFT.

ASSOCIATE WAS CLEANING LOKO TRACK HOPPER BRIDGE WHEN LARGE ROCK ROLLED OUT OF HOPPER HITTING HIS LEG. HE DIDN'T THINK IT WAS THAT SERIOUS AT THE TIME. HOWEVER, A WEEK LATER IT WAS STILL BOTHER ING HIM. HE WENT TO THE DOCTOR ON 8/19/03 AND LACERATION WAS INFECTED.

EMPLOYEE WAS REMOVING FROZEN MATERIAL, SMASHED FINGER.

EMPLOYEE WAS WATCHING ANOTHER EMPLOYEE DRIVE A PIECE OF STEEL WITH A SLEDGEHAMMER. PIECE CHIPPEDOFF STRIKING EMPLOYEE.

THE HYDRAULIC TOGGLE FOR THE JAW CRUSHER WAS BEING LOWERED TO THE GROUND THROUGH THE CRUSHER FRAME. THE TOGGLE BECAME LODGED ON A SUPPORT BEAM UNDER THE CRUSHER. THE VICTIM ATTEMPTED TO PULL T HE TOGGLE FREE FROM THE BEAM. WHEN IT SLIPPED OFF OF THE BEAM, THE TOGGLE CRUSHED HIS HAND AGAINST THE BEAM.

ATTEMPTING TO DRIVE BOWL LINER OUT OF 4' CRUSHER BOWL BY STRIKING IT WITH A HEAVY STEEL SHAFT. SHAFT GLANCED OFF THE EDGE OF THE BOWL LINER CAUSING SHAFT TO KICK OUT & SMASH THUMB BETWEEN SHAF T AND BOWL.
