WHILE INSTALLING STEEL LAGGING, THERE WAS AN UNOPENED BUNDLE OF LAGGING ON THE BASKET. WHEN THE BUCKET WAS SET DOWN, THE BUNDLE SLID FORWARD LACERATING EMPLOYEE'S RIGHT FOOT.
