THE VICTIM WAS WATCHING MINER CABLE. THE ROOF STARTED TO CAVE, HE STARTED TO RUN AND THE SHUTTLE CAR WAS MOVING OUT AT THE SAME TIME. HE SLIPPED AND FELL AND WAS RAN OVER BY THE SHUTTLE CAR.

EE LIFTED ROCK BY THE SIDE OF THE BELT. HE FELT PAIN IN HIS LOWER ABDOMEN AREA. ON 4-29 HE DECIDED HE NEEDED TO GO IN TO SEE A DR. HE WENT TO MEDICAL CENTER. THEY SENT HIM TO A CHIROPRACTOR. I T WAS DETERMINED THAT HE HAD A HERNIA. HE IS SCHEDULED TO HAVE SURGERY FOR THIS.

EE WENT T O GRAB SLACK & PULL IT OUT OF THE WAY. THE SLACKTIGHTENED & CAUGHT HIM IN THE SIDE OF THE RIB & THREW HIM INTO THE ROOF & BUMPED & CUT HIS HEAD.
