THE VICTIM WAS WATCHING MINER CABLE. THE ROOF STARTED TO CAVE, HE STARTED TO RUN AND THE SHUTTLE CAR WAS MOVING OUT AT THE SAME TIME. HE SLIPPED AND FELL AND WAS RAN OVER BY THE SHUTTLE CAR.
