Based on a shift from our annual hearing test, EE had a S.T.S. EE was then sent back for a re-test with-in 30 days on 05/31/2016. EE's results on this re-test (received on 06/13/2016) showed a S.T.S. EE is a pit loader operator hired in 2011

EMPLOYEE WAS CLEANING THE TRACKS ON THE BACKHOE. HE THEN CLIMBED BACK INTO THE HOE TO SWING IT AROUND TO CLEAN THE OTHER SIDE. WHEN HE CLIMBED BACK OUT OF THE HOE, HE SLIPPED ON THE STEP AND F ELL TO THE GROUND.
