EE was having chest pain we called the emts at that point they did not think it was a heart attack but wanted to take him to the hospital to be sure. I went to see him about 3:30pm and they still did not know any thing yet. Mon 6/20 they moved him to a different hosp, later they put in 2 stints do to a 95% blockage in 2 main arteries.

EE was filling hydraulic tank on his loader. After filling tank he dropped the 1 gal plastic jug to ground, wiped oil off his hands on rag and proceeded down the steps on loader. His hands slipped off the handrail and he fell backwards hitting his head, shoulders and back on the ground.

EE was folding up a portable conveyor for transport, when it was almost folded a undetected piece of limestone dislodged from head pulley area, striking him on the head and face. He has a skull fracture received 4 stiches in head knocked out one footh his next dr. appt. is 2/13/06 to see when he will be returning to work.

We were heating the hydraulic trailer for about five hours with a salamander heater. Employees left site about 2:30 pm with the heater heating the trailer. Got a call about 3:40 pm that fire started and fire department was called by the schoolhouse attendent that lives next door.

ACCORDING TO EE HE SAID HE WAS LIFTING A PIECE OF STEEL USED TO FABRICATE A CHUTE ON THE 2 DECK SCREEN WHEN HE TWISTED AND FELT A SHARP PAIN TO HIS BACK ()LOWER). HE REPORTED A SORE BACK AS TH E REASON HE WAS GOING HOME TO THE SCALE PERSON, WHO RELAYED HIS LEAVING TO THIS OFFICE.
