B & G Materials, LLC · Accident type, without injuries

At approximately 10:30 am, employee noticed smoke coming out of the scale house at the Pawelekville pit. Ee attempted to put the fire out with extinguisher and was unable to. EE then called 911 and safety. Falls City fire department arrived fire was extinguished. After further investigation, it appears that it was an electrical fire. No injuries occurred due to the fire.

The adjusting rods on the apron were hung up. Had hand to align washers and the apron back end dropped crushing right thumb and palm.

STANDING ON FRAME OF CRUSHER-CONE TIGHTENING A BOLT WITH A CRESCENT WRENCH WHEN WRENCH SLIPPED & EMPLOYEE SLIPPED TOO & HELD ONTO THE HYDRAULIC HOSES. HE SAYS THAT HIS HIPS TWISTED IN AN AWKWA RD POSITION WHILE HE WAS HOLDING ONTO THE HOSES.
