Tightening bolts under screen when wrench slipped off and hit back of right hand on piece of metal. Sprained back of hand and broke 3rd finger.

86 Mack cabover carrying solaire air compressor for drill, air compressor caught fire, took nearly 30 min to put out, fire department was involved, no injuries. Two employee's from Givelich Explosive were just starting to drill shot when air compressor caught fire.

Cold steel, employee climbed on top of jib laying on a crusher wheel on the ground to hook a shackle to it. Standing 3-5' off ground and slipped and fell backwards landing on a rock. Broke rib, puntured lung, & fractured 3 vertabrae.

EMPLOYEE PERFORMING PRE-SHOT INSPECTION IT WAS MUDDY AND DARK. THE MUD STUCK TO HIS BOOT AND WHEN HE WENT TO MOVE, HE FELL ONTO A POINTED ROCK ON HIS LEFT SIDE OF HIS UPPER CHEST.
