EMPLOYEE WAS USING PRESURE WASHER TO CLEAN SHOVEL, STANDING ON THE CATWALK STEPPED BACK TOO FAR AND FELL OFF ONTO HIS LEFT SIDE. BROKE #5 RIB ON LEFT SIDE.

EMPLOYEE WAS ATTEMPTING TO TURN END DUMP AROUND, PULLED DOWN A SLOPE TO BACK INTO ANOTHER ROAD. EMPLOYEE SAID BRAKE DID NOT HOLD, HE THREW IT IN REVERSE, KILLING THE ENGINE. THEN HE ROLLED DOW N THE ROAD HITTING THE POWDER BIN AND ROLLING TRUCK ONTO ITS RIGHT SIDE. BROKE RIGHT LEG AND SPRAINED ANKLE, RECEIVING CAST.
