EE SAID THAT AFTER GREASING HIS LOADER HE CLIMBED THE LADDER UP TO THE CAB OF HIS 980 G LOADER. HE LOADED THREE TRI AXEL THEN SAID HE NEEDED A MINUTE TO TRY AND WALK OFF A KINK IN HIS BACK. NE XT HE RADIED THAT HE WAS HURTING TOO BAD TO GET DOWN OUT OF HIS LOADER. HE BELIEVES HE TWISTED SOMETHING CLIMBING THE LADDER.

THE FACTORS WHICH CONTRIBUTED TO EMPLOYEE'S INJURY INCLUDE 1) WEARING LOOSE CLOTHING AROUND MOVING PARTS 2) NOT PAYING CLOSE ATTENTION TO THE TASK 3) ENGAGING THE WRONG LEVER ON THE MACHINE. ALL THREE CONTRIBUTED TO HIM FRACTURING HIS WRIST.

EMPLOYEE WAS HELPING SET UP A NEW HOPPER FOR THE CRUSHER HE WAS STANDING ON A LADDER ABOUT 6 FT UP, HE WAS TAKING A BOLT OUT OF A CROSS BRACE SO WE COULD LINE THE BOLT HOLES IN THE LEGS UP WHE N THE BOLT CAME OUT. HE FELL HEAD FIRST TO THE GROUND WHICH WAS ROCK COVERED. HE HAD A CUT OVER RIGHT EYE AND A SPRAINED NECK.

EMPLOYEE WAS CHECKING FOR OVERSIZE GETTING IN STONE PILE, AS HE WAS STANDING AT THE NW CORNER OF THE DECK SCREEN A LARGE ROCK FELL FROM THE TOP DECK OF THE SCREEN ND HIT HIM IN THE HEAD. HE DID HAVE A HARD HAT ON BUT THE ACCIDENT BROKETHE SKIN ON HIS FOREHEAD. WHEN THE ROCK SMASHED THE HARD HAT ON HIS HEAD HIS NECK ALSO BECAME STIFF.
