September 20, 2021 TX · Metal/Non-Metal laborer, blacksmith, bull gang, parts runner, roustabout, pick-up man, pitman HANDTOOLS (NONPOWERED)
Far South Mining LLC · Struck against stationary object

Employee was dislodging a rock on a discharge conveyor with a 4' crowbar. When rock was freed crowbar slipped and employee cut right ring finger on the metal guard. Machine was off, locked & tagged at time of incident. Employee was wearing gloves.
