EE was cleaning up waste in the loader when EE was finished and dismounting the machine EE reports EE was coming off of the loader and missed the last step, fell on to back and landed more on the left shoulder. EE worked for 2 days with some soreness. EE sneezed and developed pain. Went to ER and found a fractured rib on left side

Employee was checking his service truck for the day. As he exited the cab, he stepped from the top step to the lower step with his right leg, he felt a pain in his left knee.

While lowering a snubber pulley into a position where it could be broken down for repair the chain holding the drive side of the pulley slipped causing it to drop about 2" pinching employee's hand between the bearing housing and conveyor frame which resulted in his finger being fractured.

While clearing trees with a bulldozer, the employee opened the cab door to look at his track while moving when a broken tree branch struck him on his left hand. He suffered a laceration to two fingers requiring sutures. He returned to work with no restrictions.

Employee had been welding and line boring to repair a front end loader. After completion of work while cleaning up he began to experience discomfort in right eye. He went to dr 11-8-06 and had small metal fragment remove from eye.
A customer truck driver pulled his truck onto the weighout scale. He was getting out of the truck to get his scale ticket when he slipped and fell to the ground. He was later diagnosed with fractured ribs. The injured customer driver was employed by J&S, Inc.

While dismounting from the wet screen, employee slipped and caught himself, suffering a dislocated shoulder. Employee reported he had dislocated his shoulder several times in the past due to sports injuries.
Employee slipped while stepping down off a Cat 330 excavator and fell against a pickup truck parked next to the machine. He was diagnosed with a cracked rib and placed on restricted duty.

TIGHTENING TAIL PULLEY ADJUSTER NUT WITH 3/4" DRIVE RATCHET. RATCHET SLIPPED OFF OF NUT, CAUSING EMPLOYEE TO CUT PINKY FINGER ON RIGHT HAND BETWEEN RATCHET HANDLE AND ROCK ON THE GROUND.

EE WAS CLEANING THE TAIL PULLEY ON THE M-70 STACKER WITH A STEEL BAR WHEN THE BAR BECAME CAUGHT BETWEEN THE CONVEYOR BELT AND THE PULLEY. WHEN HIS HAPPENED IT PULLED HIS HAND IN THE PULLEY AND CAUSED THE INJURIES.
EMPLOYEE AND HIS SUPERVISOR WERE LOADING A RADIATOR ON THE BACK OF A PICK UP WHEN THE RADIATOR SLIPPED AND THE FAN SHROUD CAUGHT HIS WRIST AS IT FELL TO THE GROUND.
