As employee was changing a screen overhead on the Finley screen plant, a stone lodged inside his ear canal & could not be removed. Employee had to seek medical attn. from a specialist to have the stone removed.

EE was grinding on an air tank in an awkward position (standing, bent over with head tipped down), and a piece of rusted metal lodged in his right eye. EE was wearing safety glasses with side shields at the time of the accident.

USING A PRY BAR TO TURN A WHEEL IN ON A CONVEYOR. THE BAR SLIPPED CAUSING EE'S BODY TO TURN AND FOOT TO STAY PLACED THEREFORE RESULTING THE BONE TO FRACTURE.

GOT OUT OF LOADER TO SHUT DOWN PLANT. ROCK BOUNCED OFF OF SCREEN DECK & STRUCK PERSON IN THE HEAD. HARD HAT WAS NOT WORN. RECIEVED 10 STITCHES IN THE HEAD.
