As employee was changing a screen overhead on the Finley screen plant, a stone lodged inside his ear canal & could not be removed. Employee had to seek medical attn. from a specialist to have the stone removed.

EE was grinding on an air tank in an awkward position (standing, bent over with head tipped down), and a piece of rusted metal lodged in his right eye. EE was wearing safety glasses with side shields at the time of the accident.

USING A PRY BAR TO TURN A WHEEL IN ON A CONVEYOR. THE BAR SLIPPED CAUSING EE'S BODY TO TURN AND FOOT TO STAY PLACED THEREFORE RESULTING THE BONE TO FRACTURE.
