Contract employee was trying to set a poly line in a stand. When EE was trying to adjust the poly pipe EE pulled pipe over stand. It came off stand slide across manlift rail and hit EE in the ribs.

Ramsey Hill Exploration, LLC · Struck against a moving object

While operating a WA 500 loader, the operator went through a small dip at an angle causing the loader to rock. Although employee was wearing a seat belt, the operator was thrown off balance and hit their head on the side RH window.
