EMPLOYEE WAS CUTTING PIECES OF PLASTIC ON THE TABLE SAW WHEN ONE OF THE CUT PIECES VIBRATED BACK INTO THE BLADE. THE PLASTIC WENT IN THE AIR STRIKING HIM IN THE CHIN.

POWER FAILURE AT THE MAIN OFFICE. POWER CAME BACK WITHIN A MINTUES AT OFFICE. FAILURE OCCURRED AT APEX MINE SITE FUS FLEW OUT ON THE POWER SUPPLY WHICH OPERATES OUR MAIN HOIST. AT WHICH TIME W AS CALLED UTAH POWER AND LIGHT CO AND REQUESTED A BUCKET TRUCK SO WE COULD REACH THE FUSE IN ORDER TO REPLACE IT.
