Gulf Coast Limestone Inc · Over-exertion (Not Elsewhere Classified)

While attempting to climb into the cab of a 329 Cat excavator EE felt a pop in EE's left knee. EE had EE's left knee on the step and had all of EE's body weight on that knee while EE was attempting to get into the cab. Ground conditions were flat and dry. No other conditions contributed to the incident

Gulf Coast Limestone Inc · Struck by falling object

Operator had loaded a end-dump which was determined to be overloaded. The truck driver dumped some material but could not close the gate on the bed as a rock was overhanging. Operator signaled the driver to hold what they were doing and went to push the rock back into the bed. Driver dumped the air on the cylinders and the gate came down on the operator.
