Miner was operating CAT 326 long reach excavator in approx. 1ft to 3ft of water when left front track dipped and slid into aprox 15 - 20ft of water, the miner was able to escape the cab through the drivers door window and get to the surface. MSHA was notified and came to location same day. No injuries to the miner.

Attempting to remove a taper lock bushing from a large sheave. A weld on a pushing bolt broke when an airgun was applied. The spinning weld and bolt cut through his leather glove and into the palm of his right hand, which was placed at the base. The cut was approx 1 1/4" long and 7 sutures were required. He was wearing all required PPE and had done a JHA.

THE EMPLOYEE HAD HIS THUMB ON A "FORK" ON THE END OF A HYDRAULIC RAM THAT WAS BEING RETRACTED. HIS THUMB WAS CAUGHT BETWEEN THE "FORK" AND THE BODY OF THE RAM WHEN THE RAM WAS FULLY RETRACTED. THE RESULT WAS A SEVERE LACERATION AND FRACTURE OF THE BONE OF THE LAST JOINT OF HIS RIGHT THUMB.
