I.NAME HAD WALKED BEHIND HIS TRUCK TO LOOK AT A BLOCK OF GRANITE. THE TRUCK WAS PARKED ON AN INCLINE WHERE IT JUMPED OUT OF GEAR AND ROLLED BACK TRAPPING I.NAME BETWEEN THE GRANITE AND THE TRU CK AND RESULTING IN HIS DEATH.

THREE MEN WERE DRILLING HOLES IN A SECTION THAT HAD BEEN BURNE DOUT. THEY WERE WORKING FROM A PLATFORM SUSPENDED FROM A CRANE DRILLING HORIZONTALLY. A CORNER OF TH EBURNED SECTION GAVE WAY , PUSHING THE PLATFORM AWAY FROM THE ROCK ABRUPTLY, CAUSING THREE MEN TO FALL APPROX 8' LANDING ON ROCK SURFACE. TWO WERE INJURED.

THEY WERE WORKING FROM A PLATFORM SUSPENDED FROM A CRANE DRILLING HORIZONTALLY.A CORNER OF THE BURNED OUT SECTION GAVE WAY, PUS HING THE PLATFORM AWAY FROM THE ROCK ABRUPTLY, CAUSING 3 MEN TO FALL APPROXIMATELY 8 FEET LANDING ON ROCK SURFACE. 2 WERE INJURED.

ON THE LEDGE THE CHANNEL BLOCK HAD A SLOPE TO IT. HE STEPPED ONTO THE CHANNEL BLOCK. THERE WAS LOOSE GRAVEL ON THE CHANNEL BLOCK WHICH CAUSED HIM TO SLIP AND LOOSE HIS BALANCE. HE DID NOT F ALL COMPLETELY TO THE GROUND. HE CAUGHT HIMSELF, THEREFORE TWISTING AHD JURKING HIS LOWER BACK.
