The miner was traveling on a downhill slope and lost control of tractor and collided into berm at the bottom of the slope resulting in his head striking the air conditioner vent inside the cab.

Preparing to unload drill steels from truck when peg came out causing one side of drill steel to fall off truck onto steel toe of boot and roll back onto foot causing broken tip of big toe and laceration on bottom of foot due to pressure of steel on foot.
