EMPLOYEE WAS HELPING TO REMOVE A PIN ON A BRAKE BAND, AFTER CUTTING PIN, ATTEMPTED TO BLOW DEBRIS AWAY USING ACTELENE TORCH, WHEN THE GAS IGNITED, CAUSING A SMALL EXPLOSION! THUS BLOWING DIRT INTO EMPLOYEE'S FACE. ALSO A LOUD NOISE WAS AUDIBLE.
BREAKING UP LARGE COAL LUMPS IN BREAKER, USING A 6' BAR TO PRY COAL LUMPS LOOSE IN COAL BREAKER, BAR SLIPPED AND HE FELL BACKWARDS APPROX. 8' ON TO GROUND.
