LOADER OPERATOR OF A HANOMOG FRONT ENDLOADER WHILE DIGGING OUT HIT #8 CONVEYOR PUTTING A DENT IN TOP OF CAB OF LOADER THEN HE WENT DOWN TO GET A CROWBAR TRYING TO TAKE THE DENT OUT OF THE TOP OF CAB CROWBAR SLIPPED HITTING HIM IN THE FOREHEAD LOSING HIS BALANCE HE FELL HITTING THE BUCKET OF THE LOADER FALLING TO THE GROUND
