EE was dismounting the machine and was observed using 3 point contact, lost his footing and fell approx 2 1/2 feet onto his side. The steps and hand holds were intact. There was a thin layer of fresh mud on the tracks and step surface from tramming the machine 10 minutes prior to the accident.

EMPLOYEE WAS OPENING CHUTE TO CHANGE FLOW OF ROCK, SLIPPED AND FELL. WHEN HE FELL, THE CHUTE DOOR SLAMMED SHUT ON HIS LEFT HAND, CUTTING OFF TIP OF MIDDLE FINGER.
