Contractor suffered from heart attack while Onsite. Employees nearby immediately started performing CPR. Notified foreman at 11:12 AM. Paramedics was called and arrived at 11:19 AM and they took over CPR and took Contractor to hospital. Fire chief called area manager at 11:16 AM then notified safety dept 11:18 AM then MSHA called 11:23 AM.

Installing suction hose on Metso HP400 crusher. Used a band saw to cut the hose, held the hose with left hand, used a small grinder to dress up the end of the hose. EE lost grip on the hose and one of the small wires punctured glove and the tip of finger. Became infected, sought medical treatment on April 7, antibiotics were prescribed to fight the infection.

EE was climbing down ladder on a Euclid R-50 size Haul truck. When got to the next to last step he stated that he thought that he was on the last step and stepped to the ground which was further than he thought causing him to lose his balance and fall. He fell 3' landing on his left side. X-rays revealed no injury, but family Dr. put ee off until 2/8/10.

While working on a contractor's truck, employee was pulling on chain that was fastened to tailgate. Tailgate came loose. Finger got caught between tailgate and chain.
Operating a front end loader. Went in to get a bucket full at bottom of shot. Hit a real hard spot. Loader just stopped. Pain started in lower back area and down left leg.

Slipped on ice, leg struck stone. laceration to right leg.
After helping welder on cone crusher...later when went home eyes started watering and felt scratchy. Went to optician - mild flash burns to eyes.
Half straddled, half bent to move a 10" piece of channel. Never actually moved it. Reached out to move it and felt back pop. Sharp pain moved down right leg. Pain worsend as day progressed.

Was greasing sand belt when belt started moving. Flipped off feet. Grabbed cable and held on until belt was stopped. Did not seek medical treatment until 10-15-04 due to soreness. Feeling better at this time.
