WHEN POSITIONING THE DRILL, THE RIGHT REAR LEVELING FOOT SANK INTO A MUD SEAM ALLOWING THE DRILL TO OVERTURN. THE DRILL SLID OVER THE EDGE OF HIGHWALL AND THE ROCK LEDGE CRUSHED THE DRILL OPER ATORS COMPARTMENT CAUSING DEATH OF DRILL OPERATOR.

Two pieces of flooring collapsed at an elevated catwalk at the primary end of plant. The welds that secured the flooring deteriorated and loosened over time. The floor failed to support an employee that was walking the catwalk.
