WORKER TRIED TO3REMOVE A FUSE FROM A FUSE BLOCK WITH POWER ON, AS THIS WAS A 440 VOLT SYSTEM. THIS CAUSED A FLASH WHICH INFLICKED 2ND DEGREE BURNS ON THE BACK OF HIS HAND (RIGHT).

I,NAME HAD JUST ARRIVED AT JOB SITE AND WAS GOING TO INSPECT A DUMP TRUCK ON WHICH A USED DUMP BOX HAD JUST BEEN INSTALLED TO SEE IF IT WAS READY FOR SERVICE. JUST PREVIOUS AN EMPLOYEE HADST ARTED THE TRUCK AND RAISED DUMP BOX TO FULL HEIGHT; HE THEN DISENGAGED THE POWER TACK OFF AND LEFT TO WAIT ON A CUSTOMER. NORMALLY HYDRAULIC OIL DRAINS BACK INTO TANK ALLOWING CYLINDER TO RETR
