Lack of communication. Crusher was locked out, screening plant running. Victim apparently did not hear or heed warning from crusher operator, walked under crusher. Co-worker was removing rock threw off crusher struck EE as he emerged from under crusher. He was wearing hard hat.

Installing belt on crusher pulled back no witnesses notified on 7/26 was going to be checked out at hospital doctor said pulled muscles gave paid meds. sent medworks 7/31/12 sent back with RTW slip no treatment needed. Did not report at that time appeared treatment was diagnostic and preventive. Called in 8/16/12 in pain did not come to work.

Loader Op was changing cutting edges. While loosening bolts, one bolt fell out of slot. EE used his hand to hold the bolt in place, spun impact carriage bolt cut the palm of his hand. EE had 6 stitches.

EE stepped on loose rock on uneven ground at the end of the shift and fell. He got up and informed other worker he was ok. On 10/5/07 observed swelling and was sent to doctor, who released him with therapy on 11/29/07, went to another doctor and was given plastic boot.

WHEN A LARGE ROCK STOPPED THE OPERATION OF THE CRUSHER, EMPLOYEE PROCEEDED TO LOOSEN IT. ANOTHER ROCK FELL CATCHING THE LITTLE FINGER OF HIS LEFT HAND AGAINST THE STEEL OF THE SHAKER. THIS CAU SED A CUT ON BOTH SIDES OF FINGER. EMPLOYEE WAS TAKEN TO A CLINIC, RECEIVED STITCHES AND WAS RELEASED.

MINE EE WAS STANDING ATOP A SCREEN BOX CATWALK WHICH WAS BEING PREPARED FOR UNLOADING FROM LOWBOY TRAILER. A MALFUNCTION TO THE BRAKES ON THE APPROACHING LOADER CAUSED THE LOADER TO CONTACT TH E SCREEN BOX. THE SCREEN BOX TIPPED OVER, EE JUMPED TO THE GROUND AND HIS LEFT SHIN CONTACTED THE SCREEN BOX CAUSING A LACERATION, STITCHES WERE NEEDED FOR CLOSURE.

HE REALLY DOSENT KNOW WHAT HAPPEN BUT HE MUST HAVE PLACED HIS HAND ON THE BELT TO STEADY HIMSELF AS HE STEPPED DOWN TO THE WALKWAY. THE BELT PULLED HIS RIGHT HAND ARM AROUND THE PULLEY UP TO HIS SHOULDER BE
