Employee was using John Deere 350 D Excavator to load dirt and he hit a hard spot in the ground. That caused the machine to raise up on the back side. When the material broke loose, the machine fell back to the ground hard.

EE WAS BACKING UP TO PLANT TO DUMP LOAD WHEN HE NOTICED SMOKE EMERGING FROM ENGINE. HE STOPPED TRUCK, EXITED VEHICLE AND BEGAN WALKING ON PLATFORM TO CHECK ENGINE. A HANDRAIL WAS IN USE, BUT H E LOST HIS FOOTING AND FELL TO GROUND.
