Employee was taking a blown hydraulic hose off from the front of rock drill, when the fitting broke loose, EE lost EE's footing and fell into slab bedding causing left great toe fracture and left shin laceration. Area where employee was standing was slippery due to the blown fitting.

Employee kind of hopped/stepped across an area. When employee's right foot hit the ground, employee fell to the ground with pain. Doctor diagnosed as right heel muscle bruise from bone spur. Employee treated & released. Needs to use crutches as needed. Employee is working modified duty at this time, follow up appointment 06/15/2022. As of now, no lost time, no lost wages.

Cold Spring Granite Company · Absorption of radiations, caustics, toxic and noxious substances

Employee was leaving work for the day. They got into their own personal vehicle which was on the mine site, a bee followed them in at the same time, stung them on the left cheek. Employee is allergic to bees, did not have an EpiPen. Employee drove to Emergency Room for treatment. Treated and released with prescription, no work next day. Returned to work 7-29-2021.

Employee was unloading cut slabs off saw cart. Co-worker was barring slabs apart. EE dropped wooden wedge into opening, stepped back to get off saw cart, missed step, fell backwards landing on butt, back & hitting head. Due to travel & COVID didn't see doctor until 12-4-20 for imaging. 12-14-20 appt diagnosed cervical strain & right wrist strain, requires therapy. No lost time.

Employee was walking on compacted snow and ice on Tuesday, January 7th. EE slipped on the ice and landed hard on EE's right knee, then hit right elbow on the generator. EE thought EE was fine but EE's knee continued to bother EE so sought medical treatment 1-9-2020. EE was released without restrictions, however was given a prescription medication to take. No Lost Time.

Employee was coming down the ladder, slipped on ladder step, twisting EE's ankle when stepping on next ladder step. Had pain in left ankle, sought medical treatment, diagnosis acute left ankle pain. Received 14 day prescription for pain. Returned to work next scheduled work day, no lost time.

Employee was walking in quarry, stepped off small off-set on quarry's west end floor (approximately an 8-10 inch step), twisted left knee. Employee thought it would be fine but after many months, it continued to bother so he went to doctor. Doctor recommended surgery. Surgery took place March 11, 2016. First day of lost time March 14, 2016.

Employee was running drill steel into hole using steel puller, had hand on drill steel, when hand got to puller, didn't remove hand in time, finger got into puller wheels. Smashed employee's right index fingertip, amputated from bottom of nail to tip of finger.

Employee opened a wooden box used for storage, whatever they were (bees, hornets or wasps) came out of the box, one stung his left hand. By the next day, his hand, fingers and wrist were swollen so he chose to see a doctor, receiving a prescription for treatment of the sting.

Employee was barring slabs and rough backs apart using a pry bar. He felt something pop in his lower neck, upper back area. He didn't seek treatment right away but as he continued to have pain in upper back-lower neck area, he saw a chiropractor on 4-19-10, was adjusted and released, returned to work. No lost time.
