Charlottesville Plant: 70 MSHA citations Metal/Non-Metal

BECAME REPORTABLE ON 3/15/2022 AFTER EMPLOYEE HAD TO HAVE SURGERY & LOST TIME. While stepping down scale office steps, slipped on a patch of ice & fell causing injury to right shoulder.

Employee was working in the basket of the Man lift. He was using a pry bar to force a wing pulley back into the frame. As he was applying force to the pry bar it slipped from the corner of the GTU frame causing the tip of his left hand index figure to get pinched/smashed between the pry bar and the wing pulley.

The operator observed the bucket above the ground and the loader hydraulic pressure valve settings were within limits or only slightly out of adjustment which are determined not to have caused the tube to burst. We attribute the cause of the accident to material failure of the tilt cylinder tube for unknown reasons.

EE had a second degree burn on wrist while using torch. Investigation for more details is still underway.

Associate was working in crusher, ladder was let go by helper and ladder slid into crusher. Associate fell, hitting his head on rocks and damaging his arm.

Assoc was standing on back edge of welding truck platform throwing scrap metal and conveyor belts into the dumpster, bent to pick up a 3' x 42" piece of conveyor, lost balance and fell backwards breaking wrist

Injured EE was installing a 2" rigid conduit tee fitting overhead at arms reach when the fitting slipped from his hands and struck him on the forehead, under the brim of his hardhat.

Associate wetting down muck pile with water truck "cannon" older model and associate needed to be on outside of vehicle. Truck was approx. 10 feet from pile. While spraying rock 3 to 4 feet in diameter rolled down pile hit corner of platform associate standing on, broke into piece and one piece flew up hitting associate in right shin

No injury associate. 4/5/05 informed mgr diagnosed with Post Tramatic Stress Disorder from a near miss that occured on 01/27/05 - associate removing material from #3 to #4 transfer box. Associate on platform but not tied off, material came loose hit platform, assoc. fell and grabbed edge of box and his feet were on 3 x 5 angle below 4 to 5 feet off ground

ASSOC. GOT IN PICK UP PUT IN REVERSE, RELEASED PARKING BRAKE, PARKING BRAKE DID NOT DISENGAGE ASSOC. GOT OUT OF TRUCK (STILL IN REVERSE) HOOKED CABLE BACK ON PARKING BRAKE. BRAKE DISENGAGED TRUCK STARTED MOVING BACKWARD. DOOR OF TRUCK STRCK ASSOC. KNOCKING HIM DOWN & FRONT WHEELS OF VEHICLE ROLLED OVER FOOT AND ANKLE.

ASSOCIATE STEPPING DOWN FROM LADDER ONTO LOOSE FINE MATERIAL FROM CONVEYOR-TWISTED KNEE.

EE WAS REMOVING A WEAR EDGE FROM LOADER BY STRIKING EDGE WITH A HAMMER. WHEN THE EE STRUCK THE WEAR EDGE A SMALL PIECE OF METAL FLEW OFF & IMBEDDED ITSELF IN THE EE'S LEG.

EE WAS FILLING CONTAINER WITH FLUID USING HAND PUMP. FLUID SPASHED ON FACE, UNDER EYE PROTECTION, CAUSING EYES TO BURN.

STEPPED ON ROCK TO OPEN VALVE, ROCK MOVED CAUSING LEG TO TWIST.

EE WAS OUTSIDE OF THE CAB OF THE LOADER STANDING ON BUCKET ARMS CLEANING THE WINDSHIELD WHEN HIS FOOT SLIPPED.

PERSON WAS USING WRENCH TO PUT DIE ON BOLT & CHASE THREADS ALONG BOLT. WRENCH SLIPPED OFF DIE, PERSON TRIED TO CATCH WRENCH, IT PINCHED MIDDLE FINGER ON END WITH TOOL & STEEL BRACE ON TABLE.

CUT OUT 8" C-CHANNEL 7 FOOT IN LENGTH METAL TURNED AND PINCHED VICTIM'S HAND AGAISNT WOODEN FRAME CAUSING CUT ON RIGHT HAND LITTLE FINGER 1ST AND 2ND DIGITS.

EE WAS WELDING. LEFT SHIRT SLEVE CAUGHT FIRE. EE JUMPED UP FROM SEATED POSITION TO EXTINGUISH FLAME. SLIPPED ON ICE, FELL ON LEFT HIP CAUSING BRUISE.

MOVING SHIEVE IN A COUNTERCLOCKWISE ROTATION. SLIPPED AND MOVED BACKWARDS IN A CLOCKWISE POSITION. EE'S RIGHT HAND AND FINGERS WERE ENGULFED BETWEEN V-BELT AND SHIEVE.

EMPLOYEE WAS HELPING FIX A DARTITIAN IN A STONE BIN WHEN HE TRIPPED OVER A CORD AND STRUCK KNEE ON A BOARD.

CARELESSNESS ON EMPLOYEES PART.HE WAS NOT PAYING ATTENTION AND HIT HIS KNEE ON THE TRUCKS SKIP.THIS FORMED A BLOOD CLOT IN HIS LEG.

THE INJURED WAS TRYING TO STRAIGHTEN A GUARD WHILE THE PULLEY AND MOTOR WERE WINDING DOWN FROM BEING SHUT OFF BY ANOTHER EE WHEN HIS GLOVE ON HIS RIGHT HAND WAS PULLED INTO THE PULLEY.

EMPLOYEE WAS REMOVING WORM CUTTING EDGE BOLTS HIS GLOVED HAND SLIPPED WHILE EXERTING PRESSURE ON WRENCH.HIS HAND GLANCED RECEIVED 4 STITCHES IN FINGER.

EMPLOYEE WAS WASHING DOWN LOADER. WHILE STANDING ON THE GROUND HE LEANED OVER OPERATORS SEAT TO EOPERATE LEVER TO RAISE & TILT BUCKET. ONECE BUCKET LIFTED OFF THE GROUND THE LOADER MOVED FORWA RD OVER THE EMPLOYEES FOOT. THIS KNOCKED HIM TO THE GROUND. THE LOADER CONTINUED UP OVER HIS LEG AND CAME TO REST TRAPPING EMPLOYEE.

THE MEN HAD JUST MOVED THE DRILL TO A NEW HOLE WHEN THE DRILL ROD BECAME DISLODGED & FELL STRIKING THE HELPER ON HIS HARD HAT KNOCKING HIM TO GROUND. HE SUFFERED CUTS TO FACE FROM FALLING ON GROUND & TORN LIGAMENTS IN HIS LEG.

MOVING SECTION OF CONVEYOR WITH CRANE WHILE USING SLING. SLING CAME OUT OF BLOCK, CAUSING ONE END OF THE CONVEYOR TO FALL AND STRIKE EMPLOYEE IN THE LEG.
