BECAME REPORTABLE ON 3/15/2022 AFTER EMPLOYEE HAD TO HAVE SURGERY & LOST TIME. While stepping down scale office steps, slipped on a patch of ice & fell causing injury to right shoulder.

Employee was working in the basket of the Man lift. He was using a pry bar to force a wing pulley back into the frame. As he was applying force to the pry bar it slipped from the corner of the GTU frame causing the tip of his left hand index figure to get pinched/smashed between the pry bar and the wing pulley.

The operator observed the bucket above the ground and the loader hydraulic pressure valve settings were within limits or only slightly out of adjustment which are determined not to have caused the tube to burst. We attribute the cause of the accident to material failure of the tilt cylinder tube for unknown reasons.

Associate wetting down muck pile with water truck "cannon" older model and associate needed to be on outside of vehicle. Truck was approx. 10 feet from pile. While spraying rock 3 to 4 feet in diameter rolled down pile hit corner of platform associate standing on, broke into piece and one piece flew up hitting associate in right shin
No injury associate. 4/5/05 informed mgr diagnosed with Post Tramatic Stress Disorder from a near miss that occured on 01/27/05 - associate removing material from #3 to #4 transfer box. Associate on platform but not tied off, material came loose hit platform, assoc. fell and grabbed edge of box and his feet were on 3 x 5 angle below 4 to 5 feet off ground

ONECE BUCKET LIFTED OFF THE GROUND THE LOADER MOVED FORWA RD OVER THE EMPLOYEES FOOT. THIS KNOCKED HIM TO THE GROUND. THE LOADER CONTINUED UP OVER HIS LEG AND CAME TO REST TRAPPING EMPLOYEE.
