Vulcan Construction Materials, LLC · Struck by falling object

Employee was cleaning beneath the grizzly, at the dump hopper. When a clumped mass of material fell through the gap in the plate steel shielding the top grizzly, dropped approximately 15 feet, striking the employee on the top rear of their hardhat. Employee sustained a concussion without loss of consciousness and a neck sprain.

Employee was performing maintenance on 870 transmission hydraulic pump. While positioning pump on service truck bed, pump began to roll to employee's right. Employee attempted to stop pump from rolling which resulted in employee's right ring finger being pinched between pump and vise mounted to service truck bed. Employee required 15 stitches to right ring finger.

While positioning a sheet of 5'x13' 1/4" plate steel to weld, the injured employee became momentarily trapped underneath it when the steel came off of the hook used to lift it into position and fell on top of EE.

PLANT MAN STEPPED UP ONTO MIDDLE OF HANDRAIL TO LOOK INTO LOG WASHER. AS HE WAS ATTEMPTING TO STEP BACK DOWN HIS FOOT SLIPPED OFF THE MIDDLE ANGLE OF HANDRAIL AND FELL BACKWARDS ONTO HIS BACK. MIDDLE ANGLE WAS SLIPPERY DUE TO WATER SPRAY IN AREA. FELL ONTO PLATFORM NEXT TO LOG WASHER.
