On 11-16-08 EE was installing ground support to the rib in a timber set, when collaring the hole a rock fell from the back striking the miners left hand, EE's left index finger was broke from the impact.

On 11/1/08 night shift, the EE was cleaning up around the surface shop, moving wood and other materials. When EE attempted to move a portable welder and leads, he twisted as he lifted and felt a sharp pain in his lower back. The EE stopped work for the rest of the shift.

THE EMP WAS WORKING WITH THREE OTHER EMP. PLACING CRIB IN SET OF HAULAGE DRIFT TIMBER. EMP WAS STANDING ON A STAGING BOARD, PUSHING UP MATERIALS REQUESTED BY COWORKERS. HE WAS STANDING UND ANOTHER STAGING BOARD . HE MISUNDERSTOOD A REQUEST AND LOOKED OUT FORM UNDER THE STAGING BOARD. AOCK FEL FROM ABOVE AND HIT HIM IN THE FACE.
