Employee slipped on ice, fell and struck EE's head on the ground. Initial complaints were of pain in EE's head and right shoulder. Several days later, EE still didn't feel well and went to the ER. Possible head injury, will be off work at least 5 days.

On 9/27/24, employee was tightening 5/8" bolts with two wrenches when EE felt a sharp pain in the wrist. EE's wrist and hand became swollen. An x-ray revealed EE had a small piece of metal embedded in their hand from a scratch EE received the day before. Medical professionals removed the metal from EE's hand. EE's wrist was only strained.

Employee was using a hammer to remove an old bearing from a crusher shaft. A sliver of steel broke off and entered the employee's leg. An x-ray showed the metal inside employee's leg, doctors opted not to remove it at this time.

Employee was dismantling a piece of equipment on the floor of the shop, EE stuck the back of EE's hand on a nearby wooden pallet and received an 1 1/2" long sliver in the back of EE's hand. Medical personnel cut the skin to push the sliver through to remove it.

Employees were removing steel plate and rebar from a pallet that was raised on a forklift. The supervisor instructed the forklift operator to lower the pallet. Instead, the operator moved the forklift back as the supervisor was removing a piece of rebar. The rebar dislodged a piece of plate which fell onto the assisting employee's foot.

On 8/28/12, while using a wrench to turn a thread-tapper & compensating for the pain in his elbow, the wrench slipped, glanced off his safety glasses & struck him in the forehead, causing another laceration.
