54736YEE WAS WELDING A CRUSHER USING A TORCH CUTTER AND A SLAG HAMMER. AFTER FLAME CUTTING A PIECE OF METAL AND FRAGMENTS OF THE SLAG FROM THE METAL ENTERED HIS RIGHT EYE. ND FRAGMENTS OF THE SLAG FROM THE METAL ENTERED HIS RIGHT EYE.
WHILE GRINDING A PIECE OF ANGLE IRON, THE GRINDER CAUGHT THE CORNER OF THE METAL AND GRINDER JUMPED ANC CAUGHT LEFT HAND, FROM LITTLE FINGER KNUCKLE TOWARDS WRIST.
WHILE TRYING TO FREE A STUCK BOULDER, EMPLOYEE WAS POSTIONED AT THE FEEDER AREA TO THE ROCK PLANT. THE SUPERVISOR ACTIVATED THE FEEDER AND THE BOULDER DISLODGED AND ROLLED ACROSS THE EMPLOYEE S KNEE.
