EE FELL FROM THE TRACKS OF THE BACKHOE WHILE FUELING.

EE AND ANOTHER WORKER WERE LIFTING A 10 FT LENGTH OF 10 IN RUBBER PIPE. HE TWISTED HIS BACK CAUSING LOWER BACK PAIN. HE IS WORKING ON A RESTRICTED BASIC WHILE HE SEES A THERAPERT.
EMPLOYEE WAS ATTEMTPING TO CUT A BELT FOR CONVEYOR WHILE ON TOP OF SAND PILE. DR PUT IN 2 STITCHES AND SENT HIM BACK TO WORK.

EMPLOYEE WAS PUSHING ROCK FROM THE CONVEYOR BELTAND STRAINED HIS SHOULDER AS WELL AS EXPERIENCING BACK STRAIN.

EMPLOYEE WAS WELDING ON THE PUMP NOZZLE AND DID NOT HAVE IT SECURED WELL ENOUGH. IT HIT HIM IN THE ELBOW WHEN IT FELL. HE WAS INTHE SHOP WELDING AT THIS TIME.
EE WAS CLEANING OFF CONVEYOR BELT AND SLIPPED HIS FOOT STUCK IN GUARD AND HIS KNEE GOT TWISTED AS HE FELL. THE CONVEYOR BELT HAD ICE AND SNOW STILL ON IT AT THE TIME OF THE ACCIDENT. CONVEYOR BELT WAS NOT IN OPERATION AT TIME OF ACCIDENT.
HE WAS LIFTING A DOOR STANDING ON THE TONGUE OF THE SCREEN WITH ONE FOOT ON THE ENGINE DECK. HIS FEET WERE SLIPPERY AND HE TWISTED HIS CARD.
