DISCONNECT MAL-FUNCTIONED WHEN IT WAS PULLED DOWN TO OFF POSITION. THE CENTER LUG DID NOT DISENGAGE FROM THE MECHANISM. THIS ALLOWED CURRENT TO KEEP TRAVELING TO MOTOR ON ONE WIRE CAUSING THE 60 AMP FUSE TO BLOW. WHEN IT BLEW, IT BROKE 2 SAFETY LATCHES OFF THE DOOR, ALLOWING A FLASH OF FIRE TO EXIT THE BOX & BURN EE'S RIGHT ARM.
