Walking down the tower steps after starting the plant and slipped falling onto his back side.

WHILE HOOKING UP THE PRESSURE WASHER TRAILER TO THE SHOP TRUCK, THE JACK PIN STUCK. EMPLOYEE ATTEMPTED TO FREE IT WITH A HAMMER BUT HIT THE EDGE OF THE TRAILER CAUSING A SMALL PIECE OF STEEL T O FLY OFF AND INTO HIS HAND CUTTING IT AND EMBEDDING DEEP INTO THE PALM OF HIS LEFT HAND.

BELT # 7 WAS SLIPPING AND EE CLIMBED OUT ON THE CONVEYER TO APPLY BELT DRESSING ONTO THE HEAD PULLEY. EE THEN CALLED ON THE RADIO TO START THE CONVEYER. OTHER EE CALLED EACH EE TO MAKE SURE TH EY WERE CLEAR AND ALL REPLIED. HE STARTED CONVEYER, AND INJURED PLACED HIS HAND IN THE PULLEY WHILE IT WAS RUNNING.

WHILE TRANSPORTING SCREEN MEDIA TO THE PLANT FOR REPLACEMENT, A SCREEN FELL OFF THE STAGIN AREA AND DROPPED ABOUT 2' ONTO 33'S RIGHT INDEX FINGER.
EMPLOYEE COMPLAINED OF A PINCHED MUSCLE IN HIS BACK. AFTER GOING TO THE DOCTOR HE REPORTED TO ME THAT A NERVE WAS BEING PINCHED BY A DISK IN HIS SPINE. I AM NOR SURE IF THIS HAPPENED AT WORK O R NOT.

TWO EE WERE CLEANING OUT A MATERIAL SLIDE CHUTE WITH A 4 FOOT BAR. THE BAR APON IMPACT WITH THE CHUTE, BOUNCED BACK STRIKING THE SECOND EE IN THE RIGHT EYEBROW. THE CUT WAS APPROX 1" LONG.
