THE DRIVER WAS HAULING HIS FIRST LOAD OF ORE TO THE DUMP SITE. HE DIDN'T SEE A SPOTTER OR BERM BUT BACKED UP TO THE DUMP ANYWAY. THE GROUND GAVE WAY AND THE TRUCK SLID DOWN AN EMBANKMENT AND F LIPPED OVER ON IT'S TOP. OPERATOR WAS WEARING HIS SEAT BELT.
