Was stepping down from the overhead belt onto the screen deck, when slipped and fell over the side of screen deck onto the gearbox, hitting left shoulder. ER evaluation determined separated shoulder and referred to orthopedic for further evaluation.

While unloading screens from NPT truck, he strained his back while pushing screens into the loader bucket. Thought it would get better but has just gotten worse.

While cleaning gravel material from stuffer, employee attempted to push on shovel with right foot. The material gave way easier than anticipated resulting in hyper-extending the knee.
