On October 31, 2024, at approximately 2:25 PM miners were positioning a new conveyor on site, two welds on the conveyor snapped and the conveyor folded contacting a miner as EE was in position to put the locking pins in place.

The worker was helping change a roller on the crusher when the roller slipped a tang on the roller caught worker's hand causing a laceration.
