Employee was using an 8-10 lbs. hammer to assist with the removal of a tooth off a loader bucket. A piece of the tooth broke off, roughly 2-3 mm in size, and struck the employee on the back lower side of EE's right leg.

On October 31, 2024, at approximately 2:25 PM miners were positioning a new conveyor on site, two welds on the conveyor snapped and the conveyor folded contacting a miner as EE was in position to put the locking pins in place.
