EMPLOYEE WAS HAULING AWAY MATERIAL FROM WASH PLANT HE WAS DESCENDING FROM TRUCK WHEN LEFT FOOT APPARENTLY SLIPPED OFF DESCENDING STEP AS HE WAS ATTEMPTING TO STEP TO THE LOWER STEP UPON SLIPPI NG LEFT HEEL MADE CONTACT WITH THE GROUND FIRST MAKING A POPPING SOUND AT CONTACT WITH GROUND

EMPLOYEE WAS CHECKING AN OIL LEAK UNDER A DUMP TRUCK HE WAS USING. HE LEFT THE DOOR OPEN ON THE TRUCK THICKING IT WOULD CLOSE AS USUAL, HOWEVER WHEN HE CAME UP FROM UNDER THE TRUCK HE HIT THE CENTER OF THE DOOR WITH HIS HEAD CAUSING HEAD INJURY.
