LABORER WAS INFORMED BY CRUSHER OPERATOR TO CHECK CHUTE THAT WAS MOVING IN PLACE. MAN THEN ATTEPMPTED TO CLIMB TELESMITH SCREEN WHILE IT WAS IN OPERATION & LSOT FOOTING & FELL BACK INTO ADJACE NT XOVER BELT. XOVER BLETS GUARD WAS NOT IN PLACE AS IT WAS IN PROCESS OF REPAIR. IT WAS DEFORMED DURING RECENT MOBILIZATION.
