Elam Sand and Gravel · Struck by flying object

The KeyPlate wedges against the JawDie to hold it in place. A piece of angle iron was welded to the KeyPlate used as lifting eye. Using a come-along to remove the plate. The come-along applied tension to the plate the EE was hitting it with a hammer to jar it loose. Weld on the lifting eye broke, striking EE in the chin causing a cut requiring 6 stitches

Employee was on the ground working on leveling the cone crusher unit when the unit shifted. There was an affixed ladder which struck the employees left leg causing a break in the left leg. Employee was struck just above the left knee. There was damage to the cross member on the unit. The wooden block that the hydraulic jack was sitting failed causing the unit to shift.

Employee was pulling on cables to get ready for moving plant. Employee was also using a hammer during the day, using his wrist. Employee notified supervisor that his wrist was hurting.
