Technician was removing a 29.5X25 tire from the right rear position, after the tire came off the wheel, tech was setting the tire on the ground which cause the chain to loosen causing the tire to dislodge and strike the technician and caused EE to loose EE's balance and fall to the ground, landing on remote control strapped to EE's shoulders causing the fractures to the pelvis.

Employee was climbing down the ladder of the 980G front end loader when his foot slipped, due to muddy boots, and fell against the upper handrails, cracking a rib on his left side.
Worker was assisting in removing a metal pole from the ground by wrapping a chain around the pole and attaching the opposite end of the chain to the skid steer bucket. The chain slipped from the pole causing the bucket to lunge upwards, where the worker had his hand, crushing his hand in between the under side of the lab shack and the bucket of the skid steer.
