EE WAS REMOVING DRIVE SHAFT FROM TRUCK. PARKING BRAKE WAS APPLIED. HOWEVER WHEELS WAS NOT BLOCKED. WHEN THE DRIVE SHAFT WAS REMOVED THE TRUCK ROLLED FORWARD ONTO HIS LEFT LEG AND KNEE NO BROKE N BONES JUST BRUISED MUSCLES AND TENDONS.

WHILE LIFTING HEAD SECTION (20') OFF A 36" STATIONARY STACKER BELT, THE UPON SEPARATING ROTATED BECAUSE OF MOTOR AND DRIVE WEIGHT. THE ROTATION CAUSED THE CONVEYOR TO SWING BACK TO WHERE EMPLO YEE WAS ON THE 2ND SECTION OF THE CONVEYOR STRIKING HIM AND CUTTING BOTH EARS, HIS FACE AND BREAKING HIS RIGHT ARM.
