Right shoulder - heard a pop while installing new screens on screen deck. Went to Dr in Amsterdam, NY. He sent him to a Dr in Gloversville, NY. EE has been working up until 1/25/07. Has to have surgery on Feb 8, 2007. See attached work disability statement. Dates of disability 1/29/07 to 3/23/07. Up to this point no loss of time.

PULLING A STEEL PLATE FROM STEEL RACK FOR WELDING. WHILE PULLING PLATE FROM RACK, EE STEPPED SIDEWAYS TO TURN & HIS FEET SLIPPED OUT FROM UNDER HIM. EE FELL ON HIS SIDE, RUPTURING HIS SPLEEN, (WHICH AT THE TIME WAS NOT KNOWN). UPON WAKENING SAT. HE REALIZED THAT HE WASN'T FEELING WELL. HE WENT TO THE HOSPITAL & HAD SURGERY ON 1/25/03TO REMOVE HIS SPLEEN.
