REAR OF SERVICE TRUCK HAD OIL ON FLOOR AND HAND RAILS WERE SLICK. ALL OF THIS CAUSED MULTIPLE STRAINS AND BRUISES.
WAS PUSHING IN SHOT WITH DOZER. HAD SOME BACK PAIN. FINISHED SHIFT AT 5:30 AM WENT TO ER ROOM THAT AFTERNOON, DR TOLD HIM HE HAD A SLIPPED DISC. THE CO DID NOT KNOW THAT THIS WAS A JOB RELATED PROBLEM UNTIL 6/1/98
